Navigation from the Main Menu

Proceed as follows:

1  Other System
You can select the following from the list:
  • ICIS/EMT/CH/Hosp -- to select a different organization, application, or role.
  • User Profile -- to access the user's profile details.

Miscellaneous
Under Miscellaneous, you can select the following:
  • Contact Us -- displays the AIC contact numbers, for AIC care transition offices.
  • User Guide -- displays the list of user guides (PDFs) available for the application.
  • Forms -- displays the list of forms available for various ILTC (Intermediate Long Term Care) services that can be downloaded and printed. For example, service application, nursing home forms, and so on.
  • Resources -- displays the resource related information, such as mean test, medifund, and so on.
  • Search -- for more information, see Quick Search.

Quick Search (Enter search here..)
a) You can enter the name, NRIC, referral number or case number in the text box (on the right-hand side of Miscellaneous) and select task /referral/case (search criterion) from the drop-down (right-hand side) menu. If the record is found, based on the search criterion, it is displayed under the Referral List/ Task List / Case Info tab.

4  Organization selection
In the organization list, adjacent to search, you can change to any hospital to view the referrals.
By default, it displays the organization selected at the time of logging into the application.

Roles selection
In the role list, adjacent to the organization list, you can change to any role required to view referrals.
By default, it displays the role selected at the time of logging into the application.
6  Click Advanced Search
a) Referral List
You can enter the following fields for performing the advanced search on the referral list:

Field name

Description

NRIC

Enter the NRIC number of the patient.

Patient Name

Enter the name of the patient.

Filter By:

You can filter by the following criteria (check boxes):

  • Active -- to filter by referrals that are in active state.
  • Draft -- to filter by referrals that are in draft, or partially filled state.
  • History -- to filter by the referral history.

  • Call Back -- referrals that have been called back by referring source.

  • Revert Back -- referrals that have been reverted by the agency.

  • Inactive

For service providers, the filters available are Active and History.

Outcome

Select the outcome.

Service Type

Select the service type, for example, community hospital.

Ward/Clinic

Select the ward number or clinic.

Date Submitted From

Enter the date when the referral was submitted from the referral source.

Date Submitted To

Enter the date when the referral is submitted to the service provider.

MSW

Enter the MSW's name.

 
b) Task List
You can enter the following fields for performing the advanced search on the task list:

Field name

Description

NRIC

Enter the NRIC number of the patient.

Patient Name

Enter the name of the patient.

Service Type

Select the service type, for example, community hospital.

Type Select the type, for example, Dementia, RAF.

Ward/Clinic

Select the ward number or clinic.

Speciality/Discipline

Select the speciality or discipline.

Outcome

Select the outcome, such as, accept, admit, and so on.

MSW

Enter the MSW's name.
