Create Referral

   

 

Introduction

The Create Referral page allows a hospital or an organization (referral source) to create a referral using a patient's case number or NRIC. It is the first step towards referral creation.

Proceed as follows:

1   In the Create Referral window, in the Check Patient Case Info section, select one of the following:
  • Hospital Case No--if you want to search by patient's case number and create referral.
    Follow step 2
     On entering the case number, the patient's NRIC/FIN, name and citizenship are auto-populated in their respective fields.
  • Patient NRIC/FIN--if you want to  search by patient's NRIC/FIN number and referral.

2 Search by Case number
In the Hospital Case No text box, type the case number and click Next.

3 Search by Case number On selecting Patient NRIC/FIN, in the Patient NRIC/FIN text box, type the NRIC number and click Next.
The Consent For Create Referral section is displayed.

3  In the Patient Name text box, type the patient's name.  
4  In the Citizenship list, select the patient's citizenship and click Proceed.
The Consent For Create Referral page is displayed with the previously populated fields (patient name, NRIC and so on).

 		 In the Referral Type Selection section, select one of the following services:
  • Community Hospital / Transitional Convalescent Facilities (CH/TCF)
  • Non Residential (DRC/DDC/HC/SPICE/ACE/SDC/SHC)
  • Nursing Home (NH)
  • Palliative (AIC-HOME/HOSPICE)

After selecting the required service, click Proceed.
The patient's biodata page is displayed.

Note
 

The Biodata tab, Srv App tab, Medical Rpt tab and so on are displayed only after you save the patient's biodata.
