Entering the Functional Status Details

  

 

Introduction

The functional status page allows you to enter the patient's functional status (for example, mobility, feeding) for referral creation.

Entering the Functional Status Details     

Proceed as follows:

1   On the Functional Status tab, in the Previous Functional Status section, enter the patient's functional status, prior to the onset of present illness, for the following fields:
  • *Feeding
  • *Toileting (bladder)
  • *Toileting (Bowel)
  • *Mobility

For community hospital services, it is mandatory to enter the previous functional status. So, the Previous Functional Status section is expanded by default and the fields are displayed in bold.
For services like nursing homes, nonresidential and palliative, it is optional to enter the previous functional status (fields not in bold). For non-residential and palliative services, by default, the Previous Functional Status section is collapsed.

In the Current Functional Status section, enter the patient's current functional status for the following fields: 

  • Date of onset of current illness (dd/mm/yyyy)
  • *Mental status
  • *Visual Impairment
  • *Hearing Impairment
  • *Mobility
  • *Transfer

  • *Activity Tolerance

  • *Feeding

  • *Toileting (Bladder)
  • *Toileting (Bowel)
  • Respiratory Care
    For respiratory care, you can select (if applicable) the following details (check boxes):
      >Suction
      >O2 Therapy
      >Tracheostomy
      >BIPAP
      >Others

If Respiratory Care is not applicable, select NA and you are not required to enter further details.

After completing the functional status, in the Completed By section, you can update the name, designation, telephone, email and date, accordingly.

By default, the information of the user logged into IRMS is populated. The information (name, telephone number and so on) is based on the details available in the user’s profile.

When the functional status is ready, you can do one of the following:
  • Save -- to save the functional status as draft.
    You may want to use the save option, if the page is partially populated or you do not want to submit it immediately.
  • Ready -- to save and mark the draft as ready to submit (indicated by flag in orange).

The mandatory fields that are not entered properly are indicated with asterisks (**). A message is displayed on the top of the page to enter the required fields.

You cannot submit the page without completing the mandatory fields. Once the page is submitted (indicated by flag in green), you cannot modify the details.
