Entering_the_Senior_Home_Care

Introduction

The Senior Home Care page allows you to assess whether a patient has difficulty to access services around the community/home.

Prerequisites

The senior home care page is enabled when the Senior Home Care service type is selected in the Svc App page for non-residential services:

Senior Home Care (Home Help Services) > Home Nursing Service > Wound Care Only or Procedures

Entering the Senior Home Care Details

Proceed as follows:

On the Senior Home care tab, select Yes or No for the following:

*Lift landing
*Lift landing - kerb
*Food Amenities: Accessible within walking distance and manageable by applicant:
*Access to common corridor: Steps from house to corridor
*Access to kitchen - kerb / step
*Access to toilet - kerb / step
*Home modification done (installed grab bars, sitting toilet, ramps, etc)
*Transport: Available to navigate
*Access to public transport

If you want to add any other related information, you can enter it in the Other textarea.

In the Completed By section, details pertaining to the user (role) who provided information on the senior home care is displayed

The user information (name, telephone no and so on) is based on the details available in the user’s profile.

		Note
		By default, the information of the user logged into IRMS is populated. The information (name, telephone number and so on) is based on the details available in the logged-in user’s profile.

When the Senior Home Care page is ready, you can do one of the following:

Save -- to save the Senior Home Care page as draft.
This option allows you to save the draft that is partially completed or you do not want to submit it immediately.
Ready -- to save and mark the draft as ready to submit (indicated by flag in orange).

		Note
		The mandatory fields that are not entered properly are indicated with asterisks (**). A message is displayed on the top of the page to enter the required fields. You cannot submit the page without completing the mandatory fields. Once the page is submitted (indicated by flag in green), you cannot modify the details.
