Introduction

The physiotherapy report page documents the patient's rehabilitation status and progress during physiotherapy.

This page can be documented by a physiotherapist only.

 

 

 

Entering the PT Details     

Proceed as follows:

On the PT tab, enter the following details:

Field name

Description

a) Mobility  

You can select from the following list:

  • *Mobility Status
  • *Bed Mobility
  • *Ambulation
  • *Walking Aid
  • *Transfer
  • *Exercise Tolerance

Standing Balance

You can select from the following list:

  • *Static Standing
  • *Dynamic Standing

 Sitting Balance

You can select from the following list:

  • *Static Sitting
  • *Dynamic Sitting Direction
b) Muscle Strength

Select muscle strength (numeric value) for both arms and legs from the list available.

c) Special precautions and contra-indications

You can select the following (check boxes):

  • Cardiac
  • Fall
  • Postural Hypotension
  • Others
d) Care giver training done

Select one of the following:

  • Yes.
  • No
  • Not Applicable

e) Other Remarks

Enter additional comments (if any) and the caregiver training and recommendation for follow up in the textarea provided.
f) Modified Barthel Index Click Calculate MBI.

Calculate MBI

a) To calculate MBI, you can select the MBI at the time of admission and discharge under the following categories:

  • Personal Hygiene

  • Bathing Self

  • Toilet

  • Dressing

  • Bowel Control

  • Bladder Control

  • Ambulation

  • Chair/bed transfers

  • Stairs Climbing

b) There are points associated with each selection made under Admission Score and Discharge Score, respectively. They are added and displayed in the Total Score text box.

c) The Completed By section, by default, displays the information of the user logged into IRMS. You can update the name, designation, email, contact and date, if required.

d) After completing, click Save & Close.
  The OT page is displayed with the admission score and discharge score, respectively.

Hint  

Note

   The information (name, telephone number and so on) is based on the details available in the logged-in user’s profile.
4  When the PT page is ready, you can do one of the following:
  • Save – to save the PT page as draft.
    You may want to use the save option, if the page is partially completed or you do not want to submit it immediately.
  • Ready – to save and mark the draft as ready to submit (indicated by flag in orange).

Note

 

The mandatory fields that are not entered properly are indicated with asterisks (**). A message is displayed on the top of the page to enter the required fields.

You cannot submit the page without completing the mandatory fields. Once the page is submitted (indicated by flag in green), you cannot modify the details.
