Entering the Health Information Details

Introduction

The Health Info page allows you to enter the basic health information that the healthcare professional (nurse, therapist, social worker) can provide to the best of their knowledge about the patient's health. You can attach relevant documents to support this information.

Prerequisites

Referral Source can have the option to submit Health Information, only if, Home Health Care > Home Nursing > Wound Care Only service is selected.

Entering the Health Information Details

Proceed as follows:

In the Health Information section, in the *Reason for Admission textarea, type the reason for the patient's admission.

To provide information about the patient's health condition, select (if applicable) from the following check boxes:

Hypertension
Stroke
Heart Disease
Hyperlipidaemia
Dementia
Renal Disease
Diabete Mellitus
Psychiatric Condition
COPD
Infectious Disease
If you want to add any other health information, you can enter it in the Other textarea.

In the Completed By section, details pertaining to the user (role) who provided information on the nursing procedures is displayed.
You can update the name, designation, telephone, email and date, if required.

		Note
		By default, the information of the user logged into IRMS is populated. The information (name, telephone number and so on) is based on the details available in the logged-in user’s profile.

When the Health Info page is ready, you can do one of the following:

Save -- to save the Health Info page as draft.
This option allows you to save the draft that is partially completed or you do not want to submit it immediately.
Ready -- to save and mark the draft as ready to submit (indicated by flag in orange).

		Note
		The mandatory fields that are not entered properly are indicated with asterisks (**). A message is displayed on the top of the page to enter the required fields. You cannot submit the page without completing the mandatory fields. Once the page is submitted (indicated by flag in green), you cannot modify the details.
