Introduction

The biodata page allows the referral source (referring hospital or organization) to enter the patient's biodata for referral creation.

Entering the Biodata Details     

Proceed as follows:

1   On the Biodata tab, in the Referral Source section, enter the details as described in the following table.

The Referral Source section records the information pertaining to the hospital/organization where the patient has initially obtained treatment and is creating a referral for further follow up.

Field Name

Description

*Referral Source

Name of the referring hospital or institution.

By default, the organization that is selected after logging in to IRMS is populated. If you want to change to another hospital as referral source, you can change it from the menu on the top-right corner of the page.

*Referral Type

Type of Referral.

Default value: E-Referral.

*Date of Referral Submission Date when the referral was submitted by the referral source.
This field is auto-populated when the referral is submitted.
*Date of Referral Creation Date when the referral is created by the referral source.
This field is auto-populated when the referral is created.
Admission Date Date when the patient is admitted to the referral source.
Date of Discharge Date when the patient was discharged from the referral source.
*Ward/Clinic Patient’s ward or clinic number at the referral source.
Bed Patient’s bed number at the referral source.
Speciality / Discipline Speciality or discipline under which the patient has received treatment.
*Telephone No Telephone number at the referring institution to contact.
Ward/Clinic Fax Ward or clinic FAX details of the referring institution.
Class of Stay Class of stay chosen by the patient at the referral source.
2   In the Patient Biodata section, enter the patient information as described in the following table:

Field Name

Description

Case No.

Patient’s case number at the referral source.
When you create referral using case number (search by Case number), it is auto-populated under the referral section, in the biodata page.
Also, it is a key field for searching patient biodata.

*Race

Patient’s ethnic group.

*Name   Patient’s name.
*Citizenship Patient’s citizenship.
If the citizenship is not the same as mentioned in the list, you can select Others and type the citizenship accordingly.
*NRIC Patient’s NRIC number.
By default, the NRIC number provided when creating a referral (Create Referral tab) is displayed. It is a key field for searching the patient biodata.

*Religion

Patient’s religion.

*Date of Birth (dd/mm/yyyy)

Date of birth of the patient in the dd/mm/yyyy format.

Age

Age is auto-populated on entering the patient's date of birth.
*Marital Status

Patient’s marital status.

*Gender Patient's gender.
Telephone1 /
Telephone2
Patient’s telephone number available for communication. The telephone number should be of length eight digits.
If the patient has an alternative number, you can provide that in the
Telephone 2 text box.  

Language

Dialects

Languages and dialects known to the patient.

3   In the Patient’s NRIC Address section, in the *Block/Lot, *Street, *Level, *Unit and *Postal Code text boxes, type the patient’s NRIC address.
4   In the Other Information section, select the *Living Arrangement (for example, with relatives) and *Lift Landing accordingly.
5   In the Last Known Accommodation Prior to this Application section, select the patient’s residential *Status and *Type accordingly.
6  In the Discharge Destination section, select one of the following options for *Address:
  • NA -- If not applicable, you are not required to provide further details.
  • NRIC Address – If the discharge address is same as the NRIC address. Selecting this option, copies the address from the Patient's NRIC Address section to the Discharge Destination section.
  • Non NRIC Address -- If the discharge address is different from the NRIC address. Enter the *Block/Lot, *Street, *Level, *Unit, *Postal Code and *Telephone number, accordingly.
7 Under the Contact Person and/or Caregiver Information section, select one of the following:
  
a. The applicant is a destitute or person with no relative -- The caregiver related information will be hidden. Referral source cannot enter any details.
  
b. *Has Carer been identified, select one of the following:
  • Yes – Allows you to select the carer from the options displayed.
  • No – No carer is identified.

 

c. In the Contact Person and/or Caregiver Information section, enter the fields as described in the table:

The name of the contact person or caregiver.

Address

Address Line 1

Address Line 2

Patient’s contact person or caregiver’s address.
You can select the patient’s address same as the NRIC or discharge address.

Hand Phone /

Home Phone /

Office Phone

Patient’s contact person or caregiver’s phone number. You must provide at least one telephone number.

Email Patient’s contact person or caregiver’s e-mail id.
*Relationship Caregiver’s relationship with the patient.

Decision Maker

Selects patient’s contact person or caregiver as decision maker.

Carer

Selects patient’s contact person or caregiver as carer.

Note

  If you want to provide any other information pertaining to the caregiver, you can enter it in the Remarks textarea.
8 To add the contact person and/or caregiver information, click Add.
The contact person or caregivers record is added and displayed in the table.

Note

  In the record displayed, you can revisit to select the caregiver as Decision Maker or Carer.
Also, you can Delete or Modify, as required.
9 After completing the biodata, in the Completed By section, by default, the information of the user logged into IRMS is auto-populated. You can update the name, designation, telephone, email and date, if required.

Note

  The information (name, telephone number and so on) is based on the details available in the logged-in user’s profile.
10  When the biodata page is ready, you can do one of the following:
  • Save – to save the service application as draft.
    You may want to use the save option, if the page is partially completed or you do not want to submit it immediately.
  • Ready – to save and mark the draft as ready to submit (indicated by flag in orange).

Note

 

The mandatory fields that are not entered properly are indicated with asterisks (**). A message is displayed on the top of the page to enter the required fields.

You cannot submit the page without completing the mandatory fields. Once the page is submitted (indicated by flag in green), you cannot modify the details.
