Create Referral

Introduction

The Create Referral page allows the referral source (referring hospital or institution) to create a referral using the patient's case number or NRIC. It is the first step towards referral creation.

Entering the Referral Details

Proceed as follows:

On the Create Referral tab, in the Check Patient Case Info section > Search By, select one of the following search options:

Hospital Case No -- if you want to create/search referral using hospital case number.

Patient NRIC/FIN -- if you want to create/search referral using NRIC/FIN number.

		Note
		Hospital Case No format can differ from hospital to hospital. Also, it is not available for a new case or an outpatient case. Patient NRIC/FIN number should be of length 9. The format is: an alphabet (prefix), followed by 7 digits, ending with an alphabet (suffix).

Retrieve patient details by Case Number

In the Hospital Case No text box, type the case number and click Retrieve.
The referral search is performed using the case number. On finding the case, in the Check Patient Case Info section, the patient's NRIC/FIN, name and citizenship are displayed in their respective fields.

		Note
		The hospital case number cannot be used if the patient is not known to the hospital.

Note

In the table displaying the patient record, you can click the following:

View -- To view the patient details, for example, biodata, medical report and so on.
Copy -- To copy the patient details, (for example, biodata, medical report) and create a new referral.

To create the referral, click Proceed.
The Consent For Create Referral section is displayed.

Search by NRIC/FIN

In the Patient NRIC/FIN text box, type the NRIC number and click Retrieve.

		Note
		If the patient's NRIC is already registered for referral, the existing record is displayed. You are not allowed to duplicate the referral.

The Check Patient Case Info section is displayed with the details, such as, NRIC/FIN number, citizenship, and name (for Singapore citizenship).

Note

In the table displaying the patient record, you can click the following:

View -- To view the patient details, for example, biodata, medical report and so on.

Copy -- To copy the patient details, (for example, biodata, medical report) and create a new referral.

In the Citizenship list, select the patient's citizenship and click Proceed.

		Note
		You are allowed to select the citizenship as Others, for non-singaporeans NRIC/FIN numbers only. For a valid singaporean NRIC/FIN number, the patient name and citizenship are retrieved and auto-populated.

The Consent For Create Referral section is displayed with the previously populated fields (example, NRIC).
