Referral Consent

  

 

Introduction

During the referral consent, the referral source must take the responsibility to inform the patient and family that the relevant information can be sent to AIC (Agency for Integrated Care) and any third party involved.

 

Selecting the Referral Type    

Proceed as follows:

In the Consent For Create Referral section, under Referral Type Selection, select one of the following services for referral:
  • Community Hospital / Transitional Convalescent Facilities (CH/TCF)
  • Non Residential (DRC/DDC/HC/SPICE/ACE/SDC/SHC)

    You can select the following non-residential services:
          >Day Rehab and/or Day Care Service
          
    >Senior Home Care (Home Help Services)
          
    >Dementia Day Care Service
          
    >Home Health Care (Home Medical/Nursing/Therapy)
          
    >Assessment & Coordination for Enabling Services
          
    >Social Day Care
          
    >SPICE
  • Nursing Home (NH)

    You can select Yes or No for the following:
    >Patient/Family has consented to this NH application and to disclosure of the enclosed information to relevant and/or service providers to facilitate the application.
    Hint  

    Note

      If you select No, that is, if consent is not given, doctor has to enter the reasons in the medical report for the patient's inability to give the consent for the application.

 

  • Palliative (AIC-HOME/HOSPICE)

    Under Service Selection, you can select one of the following palliative services:     
    >AIC-HOME:
      AIC-Home program is available at certain hospitals only. For more information on this service, see   the list in the bottom-left corner of the page.

    >Hospice:
     You can choose the following hospice related services:

               Hospice Day Care
               
Hospice Home Care
               
Hospice In-patient

After selecting the required service, click Proceed.
The Biodata page is displayed.

The Biodata tab, Svc App tab, Medical Rpt tab and so on, are displayed only after you save the patient's biodata.
