Introduction

The social report page summarizes the patient's social and family background information, as well as the financial needs.  


Prerequisites

 

Entering the Social Report Details:

Proceed as follows:

1   On the Social Rpt tab, enter the details as described in the following table:

Each selection you make is associated with points that are added after you have completed the assessment. The points are displayed in the Total Points field.

Textarea

Description

Social background Family set-up, living arrangement, primary caregiver, main spokesperson and significant dynamics within the family and other community support received.

Functional Status and care needs

Premorbid functional status, previous care needs and care giving experiences/issues versus current status, clinical intervention to manage patient care. For example, management of behavioural issues.

Social Criteria Assessment

Assessment on caregivers's availability, willingness & competency, suitability of community-based/home-based services to support patient's care, resources for alternate care options.

Financial Assessment

 

Means of subsistence, applicant's financial resources like savings, medisave balance, ownership of property and other sources of income, main decision maker for patient's care and financial issues.

Additional Assistance

 

Assistance by hospital, community services to support patient's care post-discharge, for example, transport funding secured for patient's dialysis appointments, purchase of oxygen concentrator for patients on long-term oxygen therapy and so on.

Recommendations from the social worker.

2 Under the Financial Assistance (FA) section, enter the following details:

a) Selecting this check box enables the following fields:

Family financial information

  • *Name of immediate family member
  • *Relationship to applicant
  • *Marital Status
  • *Gross monthly income
  • *Net monthly income
  • *Stay with applicant
  • Occupation
  • Details on other source of income
  • No of dependents (Adult)
  • No of dependents (Child)


b) Select Yes or No and specify (if required) the details for the following:

  • Details of Saving, if any (includes patient's personal and joint accounts)
  • Medisave balance
  • Ownership of property (includes patient and co-ownership tied to patient's NRIC)


c) In the Remarks textarea, you can add any additional comments.


d) To add the financial information, click Add.

Gross PCI -- indicates  Per Capita Income.

Net PCI -- indicates Total Family Income, divided by number of occupants in the household.


e) In the FA Submission Date, enter the date.


f) Under SCHEME(s) selection, select Subsidy Deviation (if applicable).

3 Subsidy Deviation

Reason(s) for Appeal

a) You can select and specify from the following reasons:

  • Dependent not staying in same household and does not have any other immediate family's support
  • Multiple persons within the household requiring long term care
  • Others
  • Per capita household income has dropped recently and significantly (e.g retrenchment, change in employment, retirement)
  • Staying in a place of worship, or in (ex) employer's quarters/office
  • Staying in a property of AV above cut-off but on a rental arrangement

b) In the Details for Appeal textarea, you can provide the details.

c) To attach the supporting documents, click Attach Document.

 
4  After completing the social report, in the Completed By section, by default, the MSW's (logged-in) details are displayed. You can update the name, telephone, email and date, if required.

Note

   The information (name, telephone number and so on) is based on the details available in the logged-in user’s profile.
5  When the social report page is ready, you can do one of the following:
  • Save – to save the social report page as draft.
    You may want to use the save option, if the page is partially completed or you do not want to submit it immediately.
  • Ready – to save and mark the draft as ready to submit (indicated by flag in orange).

Note

 

The mandatory fields that are not entered properly are indicated with asterisks (**). A message is displayed on the top of the page to enter the required fields.

You cannot submit the page without completing the mandatory fields. Once the page is submitted (indicated by flag in green), you cannot modify the details.
